Well for me it goes back to how I developed the fetish.
It was a bit backward and has its origins in supposedly TV appropriate for all ages back in the day.

First I was exposed to the taboo of a woman unclothed, and a hypothetical perversion of a woman's role in reproduction.
There's a clip of the episode of an old TV show, Buck Rogers in the 25 Century, found on X Hamster with the search "Erin Gray Back in The Day"
This female colonel is having a conversation with these Little People that are supposed to be some sort of aliens with telekinetic powers.
The conversation veers to such topics as "What is a wife" and how the little men had babies.

Through this conversation the colonel explained a woman's role in child bearing, then expressed curiosity and horror as the little men stated that they had children by attending to the Queen, who would then lay eggs instead of birth live young like the humans the aliens appeared to be.
So then the aliens began to "off think" the colonel's clothes so that they could discover how this "woman" differed from their queen.

The perverse thought that sprung to my mind at the time was that the aliens were going to try to see if the colonel was capable of laying eggs, too.



So my second exposure to such lewd suggestions on a woman's role in reproduction came from a late night TV edited broadcast of Roger Corman's Humanoids from The Deep.
In the film, monsters that bore a resemblance to the Gill Man from The Creature from The Black Lagoon attacked beach goers, killing the men and mating with the women. 



Being a TV edit, the latter part with the women was cut out, but they left in the stinger at the end where a female survivor of the attacks was heavily pregnant, her belly appearing huge beneath a sheet, and then bare and pulsating with life inside. The woman was moaning, shrieking and screaming as the female doctor between her legs told her to "push".


The TV edit cut out what ended with a fish monster baby bursting from the young woman's belly, but at the moment of the "birth" the view jumped to the severed head of one of the fish monsters, sort of implying that they lived on through getting the young woman pregnant.


So it wasn't necessarily a turn on to hear a woman suffering and in pain rather than it was a vocal expression of a purely sexual pain involved in having a baby, making the act of impregnation sex seem like an extremely heavy responsibility and consequence.


So then fast forward to present day, and I see a video where a woman delivers a ten pound baby in a car, and a clothing restricted birthing, at that.


And I see a similar throbbing belly, and hear the mother's deep grunts and high pitched screams after announcing that her water has broken.
Yet through all the screaming she safely delivers this huge babe from between her trembling thighs, her only  immediate  concern for her offspring while the dad filmed the whole thing while driving, handling everything like a boss.


The sexiness for me is that all the moaning and screaming is what is considered natural for a woman giving birth, so that the previous fictional instances with the science fiction and horror shows had some basis in reality where baby making was concerned, only twisted into the perverse and unnatural.  Egg laying in particular.  It seems most pregnancy fetish fantasy videos nowadays either end with a belly burst, or a full figured gal inserting and then birthing round egg like objects.

I could only watch an acted birth in a movie or a TV show, never a real life one as that would be too much for me.

I admit that I become aroused when watching birth videos but I don't wish to see the baby at all. I watch them because I am a bit jealous as I genuinely would like to experience pregnancy and childbirth myself.
