Kind of similar to when me and my wife started dating.

1) Yes I have met a few pregnant women that knew about my fetish (we met BECAUSE of it). Most of them reacted pretty positively, they enjoyed knowing someone found them attractive when they were feeling so huge. Granted, most of these were single ladies who were looking for friendship and/or relationships, so they were open minded about that. I've never had a random pregnant lady find out just by me staring at her or anything. I feel like those are the types that would get "creeped out" by knowing about a pregnancy fetish.
2) I like to pretend my parents don't know, but honestly they must know - there's been more than one incident when I was younger where they came across stuff on the computer. I always told them something like it was "spam" or a virus, and they pretended like they bought it and left it alone. It happened with each of my folks on separate occasions, so I'm sure they discussed it with each other and figured it out. But again... in my mind I keep telling myself they can't possible know, haha.

I'm not sure if any pregnant women know, but when I was 14, my mum walked in on me while I was looking at pictures of preggos. When she asked what it was I tried to come up with a response but before I could say anything, she responded by saying something about me always telling her to watch out for pregnant ladies whenever I was younger. I calmed down and she said my secret was safe with her.

Twice when I was younger my mom nearly caught me. I was using the family computer because I didn't have a laptop at that age. The first time, I was watching some pregnancy timelapse on YouTube when she asked me what I was watching. I'm not sure how much she actually saw. No to both, but I have shown hints since I was little, but I have come close to people finding out, In High School, I had a girl in Choir class, I didn't notice she was pregnant until she was showing, Then I started well, treating her nice, I called her beautiful everyday because we walked the same way after that class. I come close to being caught out more often than I should... Twice this year alone I know I've come within a matter of seconds of someone walking in on me whilst I've had pregnancy fetish material up on my screen. One away around this is to always have several tabs open, so I can quickly switch to another (non-fetish related) the moment I hear anyone approaching. As we have carpet, footsteps are hard to detect if no one is wearing shoes so I always have to have ambient sound turned right down to almost zero.

I have never been caught but the idea of being caught fills me with dread and fear.
