Have you ever known of a mom who gave birth during 4 life decades? (IOW: gave birth as teen, 20s, 30s and 40s)

A friend of mine, and his wife have 7 children. They started early, but I don't think she was a teen. Probably 3-4 kids on her twenties - then she slowed down a bit. I think she's over 40 now and had her "last" baby a couple of years ago - so, not sure if she gave birth in three decades - but definitely deserving of an honorable mention ?.

You can give birth in 3 decades and have it only be 11 years apart.

Unfortunately not. No one who I personally know, at least 

The closest I can get to that is I've known someone to get pregnant pretty much straight after High School (18) and then again in her early 30's. I've also known one to get pregnant at the back end of her 20's and then gave birth again about a month after her 40th birthday.
