There was an old practice some doctors did, where if the baby failed to cry on its own, they would slap it's butt. The idea was that if you made the baby cry, it would take its first breaths by itself.

This lady is super fit even though she's heavily pregnant. I guess the joke is that the baby is strong, too, and that it'll fight (oread "throw hands") with the doctor if he tries to spank him.
