I'm guessing he's paying them a flat rate which includes taking the pictures and exposing the belly.

"Hey, I'm putting together a pregnancy picture collection. I'll pay you $XXXX to take X amount of pictures which will include lifting your top to show your belly."

You are not going to get random pregnant women to pose and show their bellies for you like this for free. Also, keep in mind that his success does not mean it will work the same for you. It's always risky business approaching random strangers on the street asking to take pics of them.

Lol, yeah, that's literally what I did! But the majority where casual conversations that ended up with me taking their picture.

I actually started doing this during my pickup artist phase.
