In all honesty here, I do not think it is a matter of the pregnancy, more so how much do they care to follow her. It is not like she stayed in her house for nine months the last two pregnancies. She is still a human being. This time around for whatever reason they have decided to follow her more.

She covered up a lot during her first two pregnancies. Celebrities have the ability to disappear. Any picture (and there are very few) from her first 2 pregnancies she either has big baggy clothes on or she's carrying a huge bag in front of her.
